note = "Funding Information: Significant challenges still exist for fellowship training in ACS. While there is no doubt that there is an extreme need for surgeons in acute care, Fellowship-trained ACS surgeons will have little difficulty finding work in academic, community, or rural settings. 23 It is too early to tell whether ACS Fellowships as currently constructed provide the proper training for all settings, or if programs should be tailored for specific aspects of ACS more appropriate to each setting (eg, academic, community, and rural). Furthermore, funding for Fellowship training is in question not only for ACS but for all surgical specialties. Federal funds subsidize postgraduate medical education through the Centers for Medicare and Medicaid Services for programs accredited by the ACGME, for up to 5 years of clinical training. As ACS involves training after the fifth year, no Fellowship Program qualifies for the subsidy, leaving the Programs to seek financial support from clinical revenues, hospital subsidies, and industry. In addition, industrial support has been significantly curtailed by ACGME rules changes in 2009, in response to public concern over conflicts of interest. 24,25 ",
